Timbercreek Rehab and Health Care, a 202-bed skilled care center in Pekin, was cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the fourth quarter of 2018.
The report lists the nursing homes in the state with A violations, which might have caused death or serious injury to a resident.
Timbercreek Rehab and Health Care was cited with two violations, including an A violation, in October and fined $27,200.
